Church Facilities Usage Agreement

  1. Your code is not to be shared with anyone outside of your party.

  2. Each usage must be approved in advance and appear on the church calendar.

  3. No smoking, vaping, tobacco, or alcoholic products allowed inside any church facility.

  4. Usage of the Family Life Center does not include any of the upstairs youth rooms.

  5. Lights, heat, and doors, must be checked and adjusted after every use.

  6. Trash must be taken to the white building beside the Family Life Center.

  7. Commodes checked and flushed.

  8. Any spills cleaned and large messes swept.

  9. Any tables/chairs, etc must be put back in their designated places.

  10. Items broken during your usage are your responsibility to report and replace/reimburse.
